Founding Fathers Medical House Call offers a full range of diagnostic tests and treatments comparable to an urgent care center and any other relevant healthcare services provider. Our team of doctors and nurse practitioners is always ready whenever you need primary and urgent care. You can rely on us in keeping mobile Electronic Health Records (EHRs) for accurate diagnosis, treatment, etc. These are the services we offer:
We also provide post-hospital visits within 7 days of discharge from hospitals or SNFs, chronic care management services, and remote monitoring. We also now offer concierge-home medical services for home-bound clients for a fee of $250 per month. This includes 1 home visit per month, virtual visit as needed, access to provider 7 days a week, medication management, coordination of in-home services, and diagnostic testing and vaccinations. Our Medical House Call visits with no insurance is $250.
